Where the form for risks and liability says

"I understand and agree that neither the dive professionals conducting this programme, ______________________________, nor the facility through which this programme is conducted, ___________________________"

The first space is for the name of the instructor which we may have to fill out on the first day. The second is for "Wavecrest Scuba"
